Service Description

This is a discounted, fast paced, no frills appointment designed for people who are comfortable figuring out the process on their own. In order to do this, you need to have done all of your homework, know exactly what to do with the certificate and need no assistance at all from us. You will also need to know how to operate Zoom for telemedicine, be on time, pay invoice in advance, and have your Patient Intake Form filled well in advance of the appointment (this is what is used to create your certificate). The Patient Intake Form will be sent by email after the appointment is acknowledged, but there is also a link on the Members page. Failure to meet these requirements will result in the need to reschedule your appointment and an additional $50 charge.
