Initial Certification

Service Description

Click here if this is your first time being certified for a Medical Card in Virginia and you do not have a doctor/provider who has written a referral for you to see me, or you wish to keep this information confidential from your provider. Pick this option too if another patient has referred you, and mention their name on the Patient Intake Form. We will assist with the whole process. All appointments are done online through video (Zoom or whatever you prefer)
