Initial Certification - Doctor Referral

Service Description

Click here if you've been referred by a physician/provider to receive certification for a medical card. We will need them to provide your diagnoses and a request for evaluation (they can fax or email it). We will assist you with the whole application process. All appointments are online through Zoom or your preferred video app.

Cancelation Policy

You can reschedule your own appointment by logging into the member's section. Please contact us if you have to cancel.
